Project Overview:

The Chronic Disease Logbook is a health tracking application designed for individuals with long-term medical conditions such as diabetes, hypertension, asthma, arthritis, or cardiovascular diseases. The app allows users to log, monitor, and manage their health metrics, medication schedules, symptoms, and doctor consultations in a structured and organized format.

The goal is to empower patients to manage their conditions effectively and provide accurate health data to their doctors for better diagnosis and treatment planning.
