Project Overview:

The Chronic Disease Self-Management System (CDSMS)** is a web-based platform that empowers patients with long-term health conditions (like diabetes, hypertension, asthma, arthritis, etc.) to actively monitor and manage their own health. The system integrates tools for logging vital signs, tracking symptoms, managing medications, scheduling appointments, and receiving personalized health recommendations.

This solution bridges the gap between healthcare providers and patients by enabling remote monitoring and improving patient adherence to lifestyle and treatment plans — leading to better long-term health outcomes.
